resuMen

Si la salud es un estado de completo bienestar físico, mental y social y no solamente la ausencia
de enfermedades, según la definición de la Organización Mundial de la Salud, (OMS), quienes
trabajan con personas en situación de dependencia no pueden actuar incidiendo sólo en uno
de los tres aspectos señalados por la OMS. El bienestar y la calidad de vida de estas personas
va a depender tanto del cuidado de la salud física y psíquica como de su presencia activa en
la vida comunitaria. En definitiva, se trata de cambiar el enfoque centrado en el servicio que se
presta y ponerlo en la propia persona atendida, teniendo en cuenta su biografía, sus vínculos
emocionales y sus proyectos personales. La coordinación de los servicios sanitarios y sociales
es una oportunidad para hacer realidad el modelo de Atención Integral Centrada en la Persona.

abstraCt

If health is a state of complete physical, mental and social well-being, and not only the absence
of disease, as defined by the World Health Organization, (WHO), those who working with people
in a situation of dependency they cannot act affecting only in one of the three aspects identified
by the WHO. The well-being and life quality of these people will depend on both the physical
and mental health care and its active presence in the community life. Ultimately it is changing the
approach focused on the service provided and put it in the assisted person, taking into account
their emotional ties, his biography and his personal projects. The coordination of health and social
services is an opportunity to realize model of Integrated care focused on the person.
